Family Assessment and Planning Team Facilitator
- GENERAL DESCRIPTION
- Primary responsibility is to facilitate Family Assessment and Planning Team (FAPT) Meetings. FAPT members include representatives from the local Department of Social Services, schools, Court Services Unit, and the Community Services Board. The FAPT is responsible for determining eligibility for services to help meet the needs of at risk children and families in Virginia under the Comprehensive Services Act (CSA). FAPT meetings must ensure policies and procedures are followed according to the CSA.
- ESSENTIAL JOB FUNCTIONS
- Engage and educate families on the FAPT process through the life of the case, including family rights and responsibilities
- Facilitate multidisciplinary team meetings
- Provide FAPT team members with training and support as needed.
- Demonstrate knowledge of state and local policy
- Document eligibility for all CSA cases in the case record
- Review team recommendations to ensure appropriateness of CSA funds
- Ensure required documentation is received prior to each meeting.
- Ensure all meeting participants receive copies of FAPT Plans after each meeting
- Manage all the paperwork (pre and post meetings) for FAPT meetings
- Demonstrate effective time management and organizational efficiency
- Strong interpersonal communication skills with the ability to facilitate group dynamics within a meeting setting
- Ability to apply policy to practice and also be solution focused
- Other duties as assigned
- REQUIREMENTS/PREFERENCES
Education
Required: Bachelor’s degree from an accredited college or university with major coursework in social work, psychology, counseling, or a related field or combination of training, education, and experience which demonstrates the ability to perform the work described.
Experience
Required: Some experience in the human services field and/or knowledge about available resources to include educational, developmental, social, health and court services for youth and their families. Knowledge of programs, social economic trends and behaviors that impact the delivery of human services.
Certifications/ Licenses
Required: Valid Virginia driver’s license with good driving record.
Knowledge, Skills and Abilities- Ability to write and speak clearly and effectively.
- Skills in both case management and customer service.
- Ability to analyze complex problems.
- Ability to use a personal computer with word processing, spreadsheet and related software to perform a variety of administrative tasks with reasonable speed and accuracy
- Ability to interpret laws, policies and regulations for compliance with federal, state, local and agency standards, guidelines, regulations, policies and laws.
- Ability to exercise sound judgement, discretion, tact and resourcefulness in guiding difficult conversations and coordinating services.
- Ability to adhere to professional ethics related to confidentiality.
- Ability to maintain effective working relationships with employees, school personnel, service providers and the general public.
- Ability to communicate effectively with all levels of staff, other agency officials, and the public.
Additional Requirements
Subject to a complete criminal history and Child Protective Services background search with acceptable results. Must also be able to perform the job as described in the Physical and Environmental Demands section of this job description. May be subject to DMV driving record review as required by amount of work related driving, specified by support responsibilities. May work outside of normal business hours as needed by department.
