The Shelbyville Manor is looking for dedicated caring and professional nurses to join our team. Responsible for providing direct nursing services to the residents of the facility and working in a team approach with the Nurses Aides to ensure residents are receiving said services. Prepares and documents medications and treatments which are administered to residents as prescribed by the resident's Physician. Complete all nursing documentation forms as assigned and ensure that said documentation accurately reflects services required by the resident and provided to the residents. Responsible for making sure resident's physician and family are informed of medical conditions as required by regulations and facility policies. Complete resident's admission and discharge forms and duties as required by regulations and facility policies. Work using the guidelines established from the Nurse Practice Act, company standards, policies and procedures and nursing judgment. Implement the resident plan of care and evaluate the resident response. Must maintain confidentiality and resident rights, regarding all resident/personnel information. Standard precautions and other appropriate infection control practices are utilized with all nursing interventions. Perform other duties as requested by the Director of Nursing.

Specific Duties:

 

  1. Prepare information for direct care staff regarding resident’s day-to-day needs (i.e. vital signs needed, specific activity orders, intake and output needs, etc.)
  2. Make rounds on hall and monitor resident’s condition, provide necessary medical care, and document assessments and outcome.
  3. Administer medications as ordered by physician and document.
  4. Complete treatments as ordered by the physician and document.
  5. Assure physicians and resident’s families are notified of any significant changes that occur and document.
  6. Make rounds with physician when in facility.
  7. Transcribe all physicians’ Orders and follow up as indicated by order.
  8. Complete Resident admissions and discharges.
  9. Responsible for accident/incident follow-up:
    1. Provide necessary care to resident
    2. Notify Physician and follow orders
    3. Notify family/responsible party
    4. Document all of the above in resident’s medical record

 

  1. Complete daily documentation as required:
    1. Medicare charting;
    2. Weekly/Monthly summaries;
    3. Resident condition changes or unusual occurrence;
    4. Physician notification.

 

  1. Responsible for ensuring all ancillaries used are charged out to specific resident.
  2. Assume responsibility for communicating any resident care concerns to Shift Coordinator.
  3. Communicate resident’s status to oncoming Shift Nurse. Complete shift communications sheets and distribute to concerned parties (i.e. Director of Nursing, Care Plan Coordinator, etc.).
  4. Attend required in-services and meetings.

The employee must occasionally lift and / or move / push and / or pull up to 50 lbs. Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus.  While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle or feel; reach with hands and arms; talk or hear and smell.   The employee is required to move frequently and occasionally required to stoop, kneel, crouch, climb, balance, or crawl.  The employee is frequently required to sit. Employee must be able to assist in the evacuation of residents in emergency situations.

 

Emotional stability to deal with stressful situations on a regular basis; Ability to deal with illness, death and dying.

 

Qualifications:

Education: Must be a Registered or Licensed Practical Nurse, currently Licensed in state where the facility is located
